Community Health Nurse
Permanent Part Time - 8 hours per week
We are looking for a passionate, qualified nurse to work within our Allied & Community Health as a Community Health Nurse. This position is to support the implementation of “Care for people with chronic conditions” guide across the Allied & Community Health Division. This will include developing pathways for specific conditions and service models that build client capacity to self-manage their condition.
Our ideal candidate will have:
Current Registration as a Registered Nurse Grade 2 with AHPRA. Through understanding of chronic disease management principles and models of care. Ability to work within a multi-disciplinary team to identify gaps and develop new service models. Change management skills. Previous experience working with people with chronic conditions. Previous experience working within a community health setting. Driver’s Licence and use of own vehicle from time to time may be required.
