| Position Summary |
West Gippsland Healthcare Group are seeking an experienced Social Worker to join the team on a casual basis. The successful applicant will provide services at the West Gippsland Hospital. The ideal candidate will have a degree in Social Work and be eligible for a membership with Australian Association of Social Workers.
The primary focus of the position will be to ensure a team approach to the psychosocial assessment, planning and implementation of the discharge care needs of the patient in the acute setting.
